Senior care apps for US agencies operating under more than one state's rules.

Our senior care app development USA work serves home care agencies, assisted living operators and family care products that run in one state today and several tomorrow. We build caregiver scheduling, visit notes, medication reminders and family updates on a foundation that respects HIPAA, the federal EVV mandate for Medicaid and the licensing rules each state sets for itself, so expanding into a new state means changing configuration, not rebuilding the product.

An app that works in Texas can fail an audit in New York if its records, EVV route or consent model were hard coded for one state. We plan for that difference from the first discovery call.

Built for
Multi-state home care franchises and agencies, home health groups, assisted living and senior living operators, and digital health founders building for families.
Typical timeline
Discovery fixes the first release and the first states; delivery dates are written down before design starts and revisited as each state is added.
What you own
Code, app store accounts, cloud infrastructure, configuration for every state and all care records belong to your company.

Platform capabilities

One platform, configured state by state.

Senior care in the United States is funded and regulated in layers. Medicare covers skilled home health under federal conditions of participation. Medicaid funds personal care and home and community based services through state programs, waivers and managed care plans. Families pay privately or through long term care insurance, and veterans may receive home care through VA programs. On top of that, each state licenses agencies and residences in its own way and runs its own EVV model. The variation is wide: Texas HHSC licenses home and community support services agencies, California's CDSS Home Care Services Bureau licenses non-medical home care organizations, and the New York State Department of Health licenses home care services agencies.

A national platform therefore needs a core that stays the same everywhere and a rules layer that changes per state. The capabilities below are designed that way. Most operators launch in their home state, prove the workflow with caregivers and families, then switch on additional states one at a time.

Explain your project
  • State rules engine

Licensing requirements, caregiver training rules, documentation fields, EVV model and consent wording are stored as configuration per state. When you open a branch in a new state, your compliance team reviews that state's settings rather than asking developers to fork the code, and every change is versioned with who approved it.

New states go live through review, not rebuild.

  • Federal EVV data capture

The 21st Century Cures Act requires EVV for Medicaid personal care and home health visits, covering six data points: service type, client, date, location, caregiver and start and end times. States use different models, from a single state system to open aggregators. The app captures the data once and routes it to each state's required destination.

One caregiver workflow, many state EVV endpoints.

  • Caregiver scheduling at scale

Coordinators in each branch schedule against caregiver skills, languages, availability, client preferences and payer authorizations. Head office sees capacity and open shifts across every market, and can spot a branch that is losing hours before it shows up in revenue.

    Local scheduling with national visibility.

  • Care plan driven visit notes

Each visit opens the client's current tasks from their care plan. Caregivers record what was done, what was refused and any change in condition, using structured options that make records comparable across states, with free text for detail a checklist cannot hold.

    Consistent documentation in every branch.

  • Caregiver credentials by state

Training hours, background checks, health screenings and registry listings differ by state, and so do their renewal cycles. Each caregiver's file tracks the requirements for the state they work in, alerts coordinators before anything expires and blocks assignments that would put an unqualified caregiver in front of a client.

Workforce compliance managed the same way in every market.

  • Medication reminders, clearly scoped

States draw the line between medication reminders, assistance and administration differently for unlicensed caregivers. The app's medication features follow the scope each state allows, so a caregiver in one state never sees an option their training does not cover in that state.

Caregivers act within the scope their state permits.

  • Family portal and app

Adult children, spouses and legal representatives receive visit summaries, schedules and secure messages, with access granted by the client or their representative. Many families are spread across the country, so notifications respect each person's time zone and channel preference. A health care proxy or power of attorney can be given wider access than other relatives, recorded with the date and document type.

    Families informed wherever they live.

  • Private pay billing and wallets

For private pay clients, invoices are generated from verified visits and paid by card, ACH, Apple Pay or Google Pay through a PCI DSS compliant processor. Long term care insurance claims draw on the same records, exported in the format the family's insurer asks for.

Faster payment with less back office work.

  • Senior living resident experience

For assisted living and independent living communities, residents and families see activities, dining menus, transport sign ups and maintenance requests. Staff log daily assistance and incidents in a companion app, organized by community, building and shift. Families book visits and see event photos only where residents have agreed.

A consistent resident experience across every community.

  • Enterprise reporting and audits

Executives and compliance leads see visit verification rates, late and missed visits, credential expiries and incident trends by state, branch and payer. Audit trails show who viewed or changed each record, which supports HIPAA reviews, state surveys and payer audits alike.

Audit evidence available on request, in every state.

Who this is for

Four national starting points we see often.

A senior care app development company USA operators rely on must understand that the same product meets very different rules across fifty states. We are not a law firm, so we work with your compliance counsel, but we design the product so their decisions can be applied cleanly. These are the four situations we see most often, each with a different first release and a different order of states.

01

A home care franchise system

Franchisors want a branded caregiver and family app their franchisees use everywhere, while each franchise owner holds their own state license. We build a shared product with franchise level data separation, state configuration and reporting that rolls up to the franchisor without exposing one owner's clients to another. New franchisees are onboarded with their state settings already prepared, which shortens the time from signing to first visit.

Data separation between franchisees is designed in, not bolted on.

02

A home health group expanding across states

Medicare certified agencies already chart in an EHR built for OASIS and skilled care. The gap is usually aide scheduling, family communication and Medicaid EVV in new states. We build around your EHR, integrating where its API allows, so clinicians keep their charting tools and aides get a simpler app built for quick visits.

Clinicians keep their EHR; aides get a lighter app.

03

A senior living operator with communities in several states

Assisted living rules are set state by state, and so are the records surveyors expect. Operators want one resident and family app across every community. We build a common experience with state specific forms and staff workflows, so a community in one state is not forced into another state's paperwork. Corporate teams still compare occupancy, incidents and family satisfaction across the portfolio from one dashboard.

Resident consent controls what families can see.

04

A digital health company selling to families

Direct to consumer care coordination, remote check in and medication reminder apps may sit outside HIPAA. The FTC's Health Breach Notification Rule, state consumer privacy laws and newer state consumer health data laws can still apply. We design consent, data minimization and deletion so the product can grow into new states without a privacy rebuild.

Privacy obligations follow your users into each new state.

Six phases from discovery to a national rollout.

  1. 01Discovery by video callSessions with operations, compliance, IT and frontline caregivers to understand payers, current systems and which states come first.
  2. 02State and federal requirements mapHIPAA, EVV models, licensing, privacy and accessibility duties listed per state and turned into configurable rules your counsel reviews.
  3. 03Accessible design systemComponents built for older users and tired caregivers: large type, high contrast, plain language and full screen reader support, tested with real people.
  4. 04Core platform buildCaregiver app, coordinator board, family app and admin tools built in short cycles, with the rules layer kept separate from the core.
  5. 05First state launchPilot in your home state with selected branches, validating EVV submissions, payer exports and family adoption before scaling.
  6. 06State by state expansionEach new state is configured, reviewed by your compliance team, piloted with one branch and then opened to the rest of the market.

Questions national senior care operators ask.

HIPAA, Medicaid EVV, state licensing, privacy and accessibility.

HIPAA applies wherever your organization is a covered entity or business associate. Medicaid personal care and home health visits need EVV under the Cures Act, routed according to each state's model. Licensing, caregiver training and medication rules vary by state, as do consumer privacy laws. Accessibility duties come from the ADA and, for federally funded providers, Section 504. The Area Agency on Aging network, funded under the Older Americans Act, is also a common referral partner, so many apps include a referral or resource directory by county.
